Long-Term Health Care Services
An Overview
Personal Assistance Services of Colorado (PASCO) staffs a highly experienced group of competency tested, and fully licensed Certified Nursing Assistants (CNA/C.N.A.’s) to assist with Activities of Daily Living (ADL’s) for a large number of consumers/clients living independently in the community. This long-term health care is provided 365 days a year, with reliable on-call and back-up available 24 hours a day. Understanding that consumers have many options when dealing with long-term health care, PASCO works hard on the front end to make certain that the CNA/ nursing services are appropriate and the “fit” between staff and consumer is a good one. Likewise, by offering its staff generous benefits and a higher than average salary, PASCO is able to retain employees longer, ensuring that long term nursing and CNA services can be accomplished with a continuity of care that translates into successful independent living. With continued training on the intricacies of Medicaid/Medicare long-term care rules, PASCO staff remain aware and compliant with rules and regulations.
How It Works
The process begins when the client or frequently the case manager calls the office and schedules an initial intake. This intake would consist of the consumer seeking long-term care services, meeting a PASCO nurse, and PASCO scheduler, and pinpointing expectations and needs. Questions are asked, concerns are voiced, and the nurse and scheduler would decide if the consumer and PASCO could successfully work together. A list of criteria would be factored into this decision; consumer’s current living location, any biases, intolerant or hateful beliefs the consumer possesses, safe work environment, and if the consumer would meet the skilled requirements mandated to receive long-term home health services. If deemed appropriate to receive services, clinical paperwork would be sent out, and an agreed upon start of care would be stated. A nurse would be assigned and met, to go over care-plan and schedule. PASCO “Runners” (back-up CNA’s ) would be sent as well to learn the preferred routines and care –plan, followed by a few selected CNA’s ( trying the best to use the consumers criteria) who would potentially become permanent aides after a few days of training. PASCO believes very strongly in the Independent living model, and encourages our consumers/clients that it is “client directed care” meaning that it is the responsibility of the consumer to train, or show exactly the CNA how to perform various tasks on the care-plan, so that they are completed to the consumer’s satisfaction.
